RN Care Coordinator

RN care Coordinator works as part of an interdisciplinary team to identify barriers to medical or psychosocial care as needed and appropriate in a timely manner. The RN Care Coordinator collaborates with providers, clinicians and specialist as needed for appropriate medical and psychosocial care. Provides Clinical support and acts as a resource for staff as needed and appropriate. The RN Care Coordinator provides support on clinical matters and facilitates discussions with health care providers in the facilities he/she is responsible to oversee. The RN Care Coordinator supervises the Care Navigators and Transitional Care Managers who provide care management to the patients. The RN Care Coordinator facilitates the weekly staff meetings, updates and manages the patient roster, and audits the staff's charts.
