Provide substance abuse counseling and case management for a structured Intensive Outpatient and Continuing Care service. Act as a member of the multi-disciplinary team. Provide training experiences to assigned individuals and general education regarding chemical dependency rehabilitation as assigned. Facilitate and maintain responsibility for contacting referent and significant other(s) if appropriate.
CASAC or LMSW with excellent clinical skills and an expertise in lecturing on various topics related to substance abuse. Position requires knowledge and experience in a Twelve Step facilitation model. Must have experience running groups, and participating as a member of a multi-disciplinary team. Requires skill at developing individualized treatment plans and the wide range of documentation that supports the Hazelden standard of care and an OASAS certified 822 outpatient facility. Experience with third party reimbursement required as is a minimum of three years direct experience in substance abuse treatment. Candidate will be a good team player, detailed oriented and organized.
