Housing Specialist/Case Manager - Bilingual
AIDS Center of Queens County
ACQC, a longtime social service agency that assists HIV+ individuals and their families, is seeking a bilingual Housing Specialist/Case Manager in a facility based Transitional Housing Program. Individual will manage a caseload; coordinate services in order to move clients toward self-sufficiency and optimum health, and provide independent or supportive housing placements.
•Assess and identify clients’ needs and develop collaborative service plans.
•Provide advocacy and referral services to ensure that clients receive benefits, entitlements, and medical and mental health care.
•Maintain client case records (paper and computerized) including documentation, correspondences and case notes.
•Develop and maintain professional relationships with clients in order to monitor physical and emotional capacity for independent living.
•Co-Facilitate Workshops on: substance abuse counseling, mental health, Independent Living and Housing Readiness.
•Provide individual independent living skills and housing readiness counseling.
•Identify and secure permanent independent apartments in the community and supportive housing placements.
•Make on-site room visits to clients to assess the skills required for independent housing.
•Assist with intake and admission processes
•Bachelor’s Degree with at least 2 years related experience.
•Knowledge of HIV/AIDS, addictions, homelessness and mental health
•Must have housing placement experience.
•Must be bilingual; fluent in English and Spanish
•Sensitivity to cultural, ethnic and gender orientation differences
Salary: $36,000 per annum
Location: Jamaica, Queens
