Depression Care Manager

The Depression Care Manager works as part of a team of doctors and other healthcare providers in a busy community primary care center, using the Collaborative Care model of depression care. The Depression Care Manager is responsible for managing a caseload of patients with depression in primary care, providing brief and focused interventions, and consulting with primary care providers, psychiatrists and other mental health providers as necessary, ultimately resulting in decreased depression symptoms for patients.
Bachelor's in health related field strongly preferred. Bilingual English/Spanish preferred.
