Description
Requirements: Bachelors in Social Work or related field preferred; LMSW. Working knowledge of behavioral medicine and evidence based treatments for medical and mental health conditions. Working experience with a diverse populations i.e. HIV, homeless, mentally ll, elderly, children adolescents, etc. Ability to work independently and as part of a team; healthcare and mental health experience preferred. Bi-lingual, Spanish preferred.
Responsibilities: Provide direct services, which include crisis intervention, individual, family and group care in collaboration with other members of an interdisciplinary team and completion of relevant documents, intake assessments or service planned case management services. Work in collaboration with Psychiatrists, Mental Health Clinicians and other health care personnel in patient evaluation and treatment, to further their understanding of significant social and emotional factors underlying patient's health problems. Provide referrals for long-term counseling and for other necessary services (e.g. substance abuse treatment, legal services, housing assistance). Participate in development and implementation of the department's performance improvement activities. Develop linkages with community agencies as needed for funding sources, enhancing patient care or promoting the development of social services and the Institute as a whole.
