Description
Requirements: Masters in Social Work from an Accredited University,New York State License or license pending . Experience with writing to diverse populations i.e., HIV, homeless, mentally ill, elderly, children adolescents, etc. Excellent working knowledge of behavioral medicine and evidenced based treatments for medical and mental health conditions required. Ability to work through brief patient contacts as well as to make quick and accurate clinical assessments of mental and behavioral conditions; comfortable with the pace of primary care, working with an interdisciplinary team and have strong communication skills. Ability to design and implement behavior modification techniques for treatment of selected conditions; technical knowledge of medical social work and relevant community resources, ability to develop linkage with community organizations, as well as excellent communication and human relation skill required. Healthcare and mental health experience preferred. Bi-lingual, Spanish preferred.
Responsibilities: Assess patient quarterly for medical case management needs. Provides care for patients based on age-specific needs including development of comprehensive bio-psychosocial assessments responsive to age appropriate need patient/family concerns. Utilize PHQ-9 in the assessment of patients, following agency guidelines for patients care with depression. Refer patients to education and support groups appropriate to assessed needs. Provide referrals for long-term counseling and for other necessary services (e.g. substance abuse treatment, legal services, housing assistance). Facilitate prevention and wellness self-management groups. Make substance abuse referrals by conducting initial assessment, making referral for treatment, maintain ongoing communication with treatment facility, provide relapse prevention counseling to patients who have undergone treatment. Provides care for patients based on age-specific needs including development of comprehensive bio-psychosocial assessments responsive to age appropriate need patient/family concerns. Work in collaboration with physicians and other health care personnel in patient evaluation and treatment, to further their understanding of significant social and emotional factors underlying patient's health problems. Participate in COMPASS team meetings, departmental meetings, case conferences and other relevant meetings, as appropriate
