Graham-Windham, an award winning child welfare agency is seeking a qualified professional for our NYC area location. Travel is required between sites for this position for our Bridges to Health (B2H)/Waiver Service Provision program.
The qualified candidate will supervise Waiver Service Providers (WSPs) in the daily implementation of waiver services, to meet the primary objective of protecting and promoting the health, safety and well-being of children receiving Bridges to Health (B2H) Waiver Services. Supports the work of WSPs to ensure quality assessment, education, coordination, consultation, stabilization, advocacy and appropriate provision of services for children receiving B2H Waiver Services. Collaborates with Health Care Integration (HCI) Staff to ensure appropriate delivery of services included in the child's Individualized Health Plan (IHP) and Detailed Serviec Plans (DSPs).Oversees the work of 15-25 on-call WSPs. Regularly communicates with HCIs and family members to review quality of waiver service provision. Assists in recruiting, hiring, and maintenance of competent workforce of WSPs. Participates in Treatment Planning/Case Conferences as needed. Participates in emergency (24 hour crisis intervention) coverage. Participates in Training for B2H services as prescribed by OCFS guidelines. Follows guidelines for Mandated Reporters in New York State. Performs other duties as assigned.
Master’s Degree in Social Work, Psychology, or other Human Services area. A minimum of two years of experience working with children and youth. Knowledge of foster care and ACS mandates. Knowledge of the B2H waiver program strongly preferred. Proficiency in a second language – especially Spanish - a plus. Flexibility to work evenings, extended hours and occasional weekends as necessary. Must be self-motivated, a good manager of time, have good writing and organizational skills; and the ability to work independently and collaboratively with colleagues and community based resources. Must be able to make connection to community based organizations and resources. Must be able to climb stairs and conduct recreational outings and field visits to community based partners.
