Case Manager - Scattered Site

Our Mission: To reduce the rate of homelessness among individuals who are mentally ill and chronically homeless in NYC. Supported housing is one of the most effective interventions in decreasing homelessness for some of New York City's most vulnerable populations such as individuals and families coping with mental illness, trauma/abuse, addiction and chronic illness including HIV/AIDS.
Since 2007 there has been a decrease in the chronically homeless population and a primary reason for the downward trend is the increasing use of permanent supported housing. We are doing our part to end homelessness in NYC and are currently recruiting new team members who believe strongly in affording individuals the opportunity to turn their lives around.
MTI/Scattered Site is a scattered site supported housing program which serves clients in Kings, Richmond, Queens, Nassau and Suffolk counties. Our tenants are men and women with a history of mental illness, substance abuse, HIV/AIDS and chronic homelessness who are at risk of cycling between homelessness, emergency and institutional systems without affordable permanent housing and support services. Our case managers are responsible for providing individualized service planning, advocacy, practical support, care plan coordination, and collaboration with other providers, to maintain the client's permanent housing placement.
We are looking for the following:
You: Must be intelligent, quick, compassionate, nonjudgmental, bright and hardworking with a great sense of humor. You must be dedicated, willing to work as part of a team and do whatever it takes to get the job done. Must be flexible, able to work with a diverse population of clients and staff and have a proven ability to instill hope and motivation for goal setting to clients who are recently leaving extensive periods of homelessness. Possess excellent written and verbal communication skills. Meticulous and obsessed with good record keeping is favorable, and knowledge of public benefits and related providers preferred.

We are focused, and are committed to learning and improving our work to best benefit our clients. We are a detail oriented, tech savvy team, who utilizes various means to access and manage needed information.
A Bachelor’s Degree and one year experience of case management or related experience with individuals who have a mental illness is required. Valid driver’s license is preferred, but not required.
